As a contract Physical Therapist, you will provide top‑notch rehabilitation services to patients in their homes. Utilize your expertise to assess patients' physical condition, develop personalized treatment plans, and deliver hands‑on therapy. Your compassionate approach will help patients achieve their goals and improve their quality of life.

• Minimum 1 year of licensed Physical Therapist experience (home health experience preferred).
• Strong self‑management skills and the ability to work independently.
• Responsive and timely communication through messaging.
• Proficient in documentation and ensuring timely completion.
• Excellent verbal and written communication skills with a compassionate approach.

• Active California Physical Therapist (PT) license.
• Valid driver's license, reliable transportation, and auto insurance.
• Current CPR certification and professional liability insurance.
• A physical examination was conducted within the last 2 years.
• Tuberculosis (TB) test conducted within the last 1-2 years depending on test type.
• Possession of an Employer Identification Number (EIN) with an active business license.
