We are seeking new and experienced Admissions Coordinator with wonderful customer service experience who enjoys serving and interacting with the community. Education:

* Assist the Director of Community Liaison in conducting outreach and education on facility services and availability of patient care services to professional contacts, such as physicians, hospitals, related medical professionals, and discharge planners.
* Assist the Director of Community Liaison in conducting outreach and education on facility services and availability of patient care services to professional agencies and organizations, such as public health agencies, senior citizen groups, organizations serving disabled persons, and state, county, and local medical societies, or other similar groups and institutions.
* Welcoming and facilitating new admits into the skilled nursing setting, with education and follow up as necessary.
* Visitation on routine basis with new admits, assisting in the facilitation of necessary services and coordination with all patient care departments as needed.
* Assist the Director Community Liaison in the planning, developing, organizing, implementing, evaluating, and directing of the social service programs of this facility as related to new admissions and the admissions process.
* Meet with administration, medical and nursing staff, and other related departments in planning social services, as directed and necessary for patient pre and post admission needs.
* Assist family and resident as necessary in the execution of the required admissions documents.
* Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
* Participate in discharge planning, development and implementation of social care plans and resident assessment, as needed.
* Interview residents/families as necessary and in a private setting.
* Involve the resident/family in planning social service programs when possible.
