Practice Nurse

We are looking for an experienced Practice Nurse to join our friendly practice based in Marske by the Sea, the Nursing Team works closely with the rest of the Practice, and we are proud to say we are all part of the team that works together.

16 hours per week, must be able to work Thursday and Friday

Applicants must have experience working in a primary care setting. The role will include general nursing duties, must have experience in Cervical Screening and a good knowledge of Chronic Disease Management, especially Diabetes Management. Applicants should be IT literate and possess excellent organisational / communication skills.

We are a well established forward thinking GMS practice comprising 5 GP partners, 1 Advanced Nurse Practitioner Partner, Clinical Pharmacist, Paramedic Practitioners, Practice Nurses, HCA and Phlebotomist. They are supported by an excellent reception/admin team.

Zetland Medical Practice is a training practice and we currently train GP registrars and medical students.

Job Responsibilities

To respond to the general nursing requirements of practice patients.

Be responsible for meeting the nursing care needs of patients identified by the nurse, or where care has been transferred to the nurse by the GP.

In accordance with the NMC formulate individual care plans and evaluate outcomes at appropriate levels according to need.

To evaluate the outcome of that care, making changes to the care plan for individual patients where appropriate.

To demonstrate the rationale behind the various methods chosen to meet care needs of patients, to evaluate those methods and change them as necessary.

Review and plan care for patients with long term conditions.
