Position: Mental Health Practitioner-Home and Community Focused

Mental Health Practitioner-Home and Community Focused

Mental Health Practitioners provide skills-based mental health services to children, adults, and families experiencing mental health concerns. Working under the supervision of a Qualified Mental Health Professional (QMHP), MHPs help clients build practical skills that strengthen daily functioning and overall well‑being.

What You’ll Do
  • Provide skills-based mental health services to children, adults, and families (individual, family, and group modalities)
  • Use effective, culturally responsive strategies to meet client needs
  • Meet productivity and documentation standards
  • Collaborate with Fernbrook staff and community partners
  • Seek supervision and consultation as needed
  • Participate in staff meetings, trainings, and professional development
  • Uphold Fernbrook’s mission, values, and agency policies
What We’re Looking For
  • Meets Minnesota Statute requirements for Mental Health Practitioner
  • Strong ethical judgment and professional boundaries
  • Ability to manage time, schedules, and deadlines independently
  • Effective written and verbal communication skills
  • Openness to feedback and commitment to learning and growth
  • Basic computer proficiency or willingness to learn
  • Reliable transportation and ability to pass a background check
