Positive Support Analyst – Sevita

• Ensure support plans and services adhere to Individual Plans, quality standards, and regulatory requirements.
• Conduct structured assessment interviews with parents or care providers.
• Observe individuals in home, school, or community settings.
• Review social history, complete functional assessments, and establish behavior baselines.
• Write and submit assessment summaries and recommended behavior support plan components to the interdisciplinary team or operations leader.
• Advocate for the human and civil rights of individuals by presenting plan information to review committees.
• Document progress and activity, review records, and maintain confidentiality in billing and service documentation.
• Maintain professional relationships with individuals, families, guardians, and case managers; uphold company Customer Service Standards.
• Report alleged abuse or neglect following internal and external standards.
• Support staff in implementing Individual Support Plans and conduct orientation as needed.
• Assist with daily living activities such as personal care, housekeeping, recreation, transportation, and financial management.
• Accompany individuals to medical appointments and administer medications as assigned.
• Monitor health, communicate concerns with nurse or supervisor, and follow health care directives.
• Transport individuals safely and report incidents and safety concerns to authorities.
• Participate in safety drills and comply with all safety policies and training.

• Bachelor’s Degree in a human services field.
• Two years of related experience.
• Training in behavior modification techniques or experience providing behavior management treatment.
• Knowledge of specialized populations such as developmental disabilities, brain injury, or mental health.
• Reliable, responsible, and caring nature with teamwork ability.

• Valid driver’s license, car registration, and auto insurance if providing transportation.
• All state‑required training and certification completed on time.
