Overview
Unit Based Pharmacist - Oncology at Riverview Medical Center, Red Bank, NJ. The Oncology Unit Based Pharmacist is a patient-centered, unit-based model of care that integrates pharmacists with responsibility for both the clinical and distributive activities of the pharmacy department. Pharmacists are actively engaged in medication management with focus on medication utilization assessment, pain management, and antimicrobial stewardship, and exhibit ownership and accountability for the medication-use process.
The Oncology Unit Based pharmacist collaborates with physicians and the interdisciplinary team to support the multiple oncology practices within the institution.
- Validate medication reconciliation: reconcile medications upon admission and prior to discharge to ensure continuity of care.
- Perform patient assessment and counseling: develop individualized treatment and monitoring plans; educate patients about new medications; counsel on high-risk medications or complex regimens at discharge.
- Facilitate discharges: identify barriers to medication adherence and collaborate with the interdisciplinary team to address them prior to discharge.
- Collaborate with doctors to optimize cost-effectiveness and evidence-based care: review and validate orders before the first dose, monitor patient medication profiles daily, and participate in patient care rounds.
- Drive improved outcomes: work to reduce 30-day readmissions, improve patient safety indicators, and enhance HCAHPS scores.
- Adhere to organizational competencies and standards of behavior.
- Other duties and/or projects as assigned.
Knowledge, Skills and Abilities Required:
- Bachelor’s Degree from an accredited college of Pharmacy.
- Strong interpersonal skills; able to work in a team and independently.
- Excellent written and verbal communication skills.
- Registered Pharmacist License in New Jersey.
- Advanced Cardiac Life Support (ACLS) Certification within 6 months of entering position.
- Registered Pharmacist License.
- ACLS Certification within 6 months of entering position.
- PGY2 and/or more than 5 years of Clinical Oncology experience.
- Board Certified Oncology Pharmacist (BCOP).
- Practice experience in an oncology inpatient setting.
Minimum rate of $ Annually. The posted rate of pay is a reasonable good faith estimate of the minimum base pay for this role at the time of posting and may not reflect the full value of the total rewards package.
