Job Summary
The post-holder works as an expert clinical practitioner and is responsible for the delivery of patient-focused programmes of care. To provide real time triage and collect information from GPs, ED, and Ambulance Paramedics to ensure the patient is referred onto the correct pathway with internal and external stakeholders through accurate reporting, including situation reporting and escalation management systems.
Key Responsibilities- Proficient at triaging a patient’s condition through a referral process and ensuring the correct admission/admission avoidance pathway is followed, maintaining the best patient experience.
- Understand the ED 4-hour standard and how the SPOA can support this.
- Communicate effectively with ED, GPs and departments to ensure collaborative working and convey complex, clinical and confidential information.
- Take appropriate action relating to complaints and incidents and manage them effectively.
- Act as a competent leader who embraces change and supports the team to expand knowledge and confidence within the SPOA team; act up as Senior SPOA coordinator if required.
- Establish responsibility for safe and effective triage of patients referred to the Trust through Netcall; work with Pathway teams to ensure capacity and accurate information collection.
- Work with the Site Team to understand flow and support patient admission flow and avoidance of unnecessary admissions.
- Attend daily operational meetings to discuss capacity, bed availability and discharge projections; liaise with ward teams to ensure beds for emergency patients are available in a timely manner.
- Support monitoring systems to identify causes of bed shortages and facilitate escalation as needed.
- Assess and advise on referrals and care to provide best clinical pathway options.

- Registered Nurse or Registered Paramedic (Band
7) - Acute setting experience
- 1 year as a Band 7
- Educated to a minimum of 1st degree level, or working towards it
- Qualification in patient history taking
- Experience of successful Change Management
- Ability to think laterally, creatively and innovatively
- Good IT skills
