Health, we work in communities to improve health, reduce inequalities and support people to make lasting lifestyle changes. We are looking for a motivated and compassionate Wellness Coach to join our Berkshire team and help deliver the NHS Health Check Programme in West Berkshire.

About the Role

You’ll work directly with local residents to:

  • Deliver NHS Health Checks in community and GP settings
  • Identify cardiovascular risk and support lifestyle change
  • Motivate people around smoking cessation, weight management, physical activity and alcohol use
  • Set realistic goals and provide ongoing encouragement
  • Work closely with GP practices and community partners

This is a varied, people-facing role where no two days are the same, and every interaction makes a difference.

Main Duties

Support the delivery of the service by triaging, assessing and empowering people at risk of heart disease, enabling and motivating them to address lifestyle issues such as stopping smoking, achieving a healthy weight, increasing physical activity and reducing alcohol consumption.

Undertake NHS health checks and general health assessments using the Integrated Health System (IHS) in multiple local community settings, supporting people to make better lifestyle choices and take control of their health, with the option of referring them to a local lifestyle service where appropriate.

Work closely with GPs and other health professionals, community and voluntary organisations to maximise reach to West Berkshire residents.

Assess and interpret clinical information regarding referred patients, maintaining confidentiality and data protection.

Identify realistic goals, monitor and evaluate progression, and continually motivate to increase adherence to the programme.

Interact with referred patients, record details, monitor adherence and provide individual support and advice. Ensure all sessions and activities are monitored in line with agreed targets.

Maintain and regularly update patient information electronically and in paper format to support timely monitoring and evaluation.

Receive and provide sensitive information to and from healthcare professionals, including primary & secondary care.

Motivate, support and reassure patients, considering barriers to change.

Maintain accurate records and information systems in line with Solutions4

Health policies.

Assist Project Manager(s) in monitoring, evaluating and reporting on the effectiveness of programme interventions.

Establish and maintain health and safety standards within all programmes, ensuring all activities are risk assessed and undertaken with due regard to regulations.
