Job Title: Registered Nurse Assessment Coordinator (RNAC)

Job Summary:

We are seeking a highly dedicated and compassionate Registered Nurse Assessment Coordinator (RNAC) to join our dynamic team at Wyomissing Health and Rehabilitation Center in Reading, Pennsylvania. As a key member of our healthcare team, you will be responsible for conducting comprehensive assessments of patients' physical and emotional needs, and coordinating their care with our interdisciplinary team.

Registered Nurse Assessment Coordinator (RNAC) Responsibilities:

• Conduct thorough assessments of patients' medical, social, and emotional needs, and develop individualized care plans to address their unique requirements.

• Coordinate with physicians, therapists, and other healthcare professionals to ensure seamless care delivery and effective communication with patients and families.

• Develop and implement effective interventions to promote patient independence, safety, and well-being.

• Collaborate with the care team to identify and address potential risks and concerns, and implement strategies to prevent hospitalization or rehospitalization.

• Maintain accurate and up-to-date records of patient assessments, care plans, and progress notes.

Registered Nurse Assessment Coordinator (RNAC) Requirements:

• Current Pennsylvania RN license

• Prior years of experience as a Registered Nurse, with a focus on geriatric care and assessment

• Strong assessment and communication skills, with the ability to work effectively with patients, families, and healthcare teams

• Ability to work in a fast-paced environment and prioritize tasks effectively

• Strong organizational and time management skills, with excellent attention to detail and accuracy
