RN Case Manager – Reading, MA

• Assess, plan, implement, and evaluate hospice patient care across multiple settings.
• Develop individualized care plans in collaboration with interdisciplinary team members.
• Perform ongoing patient assessments considering physical, psychosocial, and spiritual needs.
• Provide education and support to patients and families, including bereavement follow‑up.
• Supervise ancillary staff and handle specialized procedures, such as venous access management.
• Maintain accurate, timely documentation supporting quality care and regulatory compliance.
• Participate in 24/7 on‑call rotations, providing coverage during holidays and off‑hours.

• Registered Nurse (RN), licensed in Massachusetts (good standing).
• Minimum of three years of diverse nursing experience, including at least one year in an acute setting.
• Strong assessment skills and the ability to respond effectively to patient and family needs.
• Knowledge of hospice principles and compassionate care delivery.
• Valid driver’s license with insured vehicle in good condition.
• Excellent communication, teamwork, and organizational skills.
• Ability to work independently, be flexible, and adapt to changing patient needs.

• Hours:
  
  Monday through Friday, Day Shift.
• Schedule options: 20/20 or 24/16 hours weekly; also available in 32 or 40 hours.
• Compensation: $45 – $60 per hour, based on experience and schedule.
