Social Services Assistant

Responsibilities

• Collaborate with patients/residents and their families to provide support and resource information.
• Respond to issues identified by patients/residents and their families.
• Facilitate patient/resident transfer within the nursing center to ensure a seamless transition and adjustment.
• Document patient/resident transfers and adjustments accordingly.
• Participate in the health care decision-making process under the direction of the Social Services Director
• Assist residents to and from needed outside physician appointments.

• High School diploma required.
• Associate degree in human services preferred.
• Minimum of two years' experience in long-term care.
• CPR certification required.
