About the Work
Field‑based RN Care Manager – travel to patients’ homes in the Rawlins, WY area. Work Monday–Friday day shifts only. No major holidays, no on‑call.
About the RoleArkos Health seeks an RN Care Manager to work closely with patients and families to ensure a smooth transition following discharge from any facility. The RN CM provides transitional and intensive nurse care management services for the entire network and works collaboratively with the health plan, leadership, and department coordinators to achieve the highest quality patient satisfaction.

- Work directly as a member of the Arkos Health Interdisciplinary Team
- Implement and update patient’s plan of care as needed
- Coordinate comprehensive care, obtaining referrals to other services (Home Health, Palliative Care, Social Services) to prevent acute‑care utilization
- Maintain case load based on client needs and contractual obligations
- Conduct face‑to‑face visits at a minimum of every 2 weeks for assigned patients; increase visits as needed
- Complete Clinical and Social Assessments (CSA) to develop care plan on initial visit
- Communicate with patients’ providers and specialists regarding patient status and changes in condition
- Engage with Arkos Health leadership and contracted providers to resolve changes, complaints or complications through weekly ICT meetings
- Complete medical reconciliation for all transitional care patients within 30 days of initiating TCM contact
- Current, unencumbered, Registered Nurse license and graduation from an accredited RN program
- Current BLS/CPR certification
- Current TB test negative every year
- Hepatitis B vaccine/titer proof (company may offer or accept declination)
- Current flu shot or waiver when mask policy applies
- In‑depth knowledge of health‑care services and medical terminology
- Excellent communication, critical thinking and time‑management skills
- Experience developing and revising care plans based on patient needs (preferred)
- Experience working in a managed‑care or hybrid field‑based environment (preferred)
- Strong technology skills and experience with electronic documentation tools
- Experience with disease‑management programs (plus)
