This role is recognized as an Advanced Practice Registered Nurse and Independent Licensed Practitioner by the Wyoming State Board of Nursing, and care is provided within the scope of practice outlined by applicable state law, licensing and regulations.
Essential Duties and Responsibilities- Perform in‑home history and physical assessments appropriate for age, gender and clinical presentation, including acute, palliative, follow‑up or home‑health evaluation visits.
- Demonstrate knowledge of etiologies, risk factors, pathophysiology and presentation of acute and chronic conditions.
- Conduct thorough review of member records to support diagnosis and care plan development.
- Develop, implement, evaluate and adjust treatment plans as needed.
- Make medication recommendations and adjustments when appropriate.
- Maintain accurate, complete and timely documentation in the electronic health record, including correct diagnosis codes and status of conditions.
- Demonstrate understanding of the Risk Adjustment Factor (RAF) and Hierarchical Condition Category (HCC) model within 30 days of hire.
- Demonstrate understanding of Quality STAR/HEDIS measures within 30 days of hire.
- Serve on a rotation of telephonic on‑call coverage after hours.
- Current and valid State of Wyoming RN license required.
- Bachelor’s degree in Nursing and Wyoming Advanced Practice Nursing license required;
Master’s degree preferred. - Board certification from a nationally recognized certifying body for Nurse Practitioners required.
- Unrestricted prescribing and dispensing privileges with DEA licensure required.
- Minimum of 1 year experience in an ambulatory or community care setting or equivalent clinical experience.
- Experience working with adult, older adult and geriatric populations preferred.
- Experience and understanding of population health management preferred.
- Strong problem‑solving and prioritization skills.
- Flexibility and adaptability to shift assignments based on organizational needs.
- Driving is required; current driver’s license and proof of insurance required.
