• Provide skilled nursing care in clients’ homes (e.g., medication administration, wound care, vital signs)
• Support clients’ emotional and physical well‑being through genuine connection
• Maintain clear documentation and communicate updates to the care team
• Educate families and clients on health conditions and self‑care practices
• Assist in hands‑on Care Professional training and ensure care compliance

• Current LPN license in South Dakota (RNs welcome to apply too!)
• Compassionate, patient‑focused approach to care
• Strong communication and interpersonal skills
• Ability to work independently and collaboratively
• Reliable transportation and willingness to visit client homes
