- Provide skilled nursing care in clients’ homes (e.g., medication administration, wound care, vital signs)
- Support clients’ emotional and physical well‑being through genuine connection
- Maintain clear documentation and communicate updates to the care team
- Educate families and clients on health conditions and self‑care practices
- Assist in hands‑on Care Professional training and ensure care compliance
- Current LPN license in South Dakota (RNs welcome to apply too!)
- Compassionate, patient‑focused approach to care
- Strong communication and interpersonal skills
- Ability to work independently and collaboratively
- Reliable transportation and willingness to visit client homes
