Responsibilities- Interview patient to obtain a complete medication history, update allergy information, and complete patient demographics when needed.
- Communicate effectively within the organization and with outside sources to determine all current and active medications prescribed to the patient.
- Serve as an important link in the communication chain, ensuring information is transmitted between nursing personnel, providers, pharmacists, other hospital personnel, patients, and family members.
- Evaluate medication, dose, route, frequency, duration of therapy, and adherence.
- Accurately document current medication history for physician reconciliation and allergy information.
- Participate in hospital and/or departmental meetings relevant to the practice area.
- Formal working knowledge; equivalent to an Associate's degree (2 years college); requires knowledge of a specialized field.
- 3‑5 years of experience.
- PTCB Certified. Maryland Board of Pharmacy Technician Registration.
