Simple Solutions Psychotherapy - Founded June of 2020. We are a community-based organization providing therapy (individual, family, couples, & groups), after-school programs, mental health assemblies, organizational workshops, juvenile detention centers, case management and more! We service California residents via insurance or cash pay. Services are provided via telehealth and in-person in English and Spanish Language and based on specifics of contracts.

At Simple Solutions Psychotherapy we believe "It's SIMPLE people just make it complicated." We seek to assist PsyD practicum students in gaining knowledge and experience toward assisting the community with mental health services.

We’re Hiring: CHW Supervisor (Community Health Worker Supervisor)

Are you passionate about supporting community health and leading teams that make a real impact? We’re looking for a CHW Supervisor to oversee and support a team of Community Health Workers who provide outreach, engagement, and care coordination to high-need community members.

This role is perfect for someone who is mission-driven, organized, trauma-informed, and ready to lead a growing team under CalAIM ECM/Community Supports programs.

What You’ll Do

Lead & Support a CHW Team

• Supervise, coach, and mentor CHWs.
• Conduct weekly team meetings and 1:1s.
• Provide field supervision and ensure safety and service quality.

Manage Program Operations

• Ensure documentation accuracy and timely completion.
• Monitor caseloads and productivity benchmarks.
• Support workflow improvements and program updates (ECM/Community Supports).

Enhance Member Care

• Assist with escalated or complex cases.
• Collaborate with nurses, social workers, health plans, CBOs, clinics, and shelters.
• Promote culturally responsive, trauma-informed services.

Drive Quality & Reporting

• Track engagement, productivity, and program metrics.
• Compile reports for leadership and health plans.
• Train CHWs on documentation and compliance standards.

Grow Your Team

• Provide ongoing training and skill development.
• Identify growth areas and support professional development plans.

What We’re Looking For

• 2+ years as a CHW, care coordinator, case manager, promotor/a, or similar role.
• 1+ year of lead or supervisory experience.
• Knowledge of social determinants of health & community resources.
• Experience with Medi-Cal populations or CalAIM ECM (or willingness to learn).
• Strong communication, problem-solving, and organizational skills.
• Valid driver’s license & reliable transportation.

Preferred: Lived experience, bilingual (Spanish, Mandarin, Vietnamese, Tagalog, Arabic), experience with EHRs or care management platforms.
