LVN Case Management

Simple Solutions Psychotherapy - Founded June of 2020. We are a community-based organization providing therapy (individual, family, couples, & groups), after-school programs, mental health assemblies, organizational workshops, juvenile detention centers, case management and more! We service California residents via insurance or cash pay. Services are provided via telehealth and in-person in English and Spanish Language and based on specifics of contracts.

At Simple Solutions Psychotherapy we believe "It’s SIMPLE people just make it complicated." We seek to assist PsyD practicum students in gaining knowledge and experience toward assisting the community with mental health services.

The Licensed Vocational Nurse Case Management will perform several tasks pertaining to client care as indicated and expected through acquired contracts. These duties usually involve working with their clients directly. The caseload of Licensed Vocational Nurses may vary.

• Conduct blood pressure screenings for enrolled members.
• Support Enhanced Care Management (ECM) members with complex medical conditions and complete medical reconciliation in collaboration with pharmacy as available.
• Engage members and support/encourage member activation towards the achievement of health goals.
• Track behavioral health outcome measures in the web-based care management platform.
• Champion healthy lifestyle changes.
• Receive, identify, and follow up treatment and medication alerts.
• Reach out to members to identify health outcomes and for client care tracking.
• Promote a collaborative and effective working environment within ECM by ending in evidence-based communication strategies.
• Strong sense of diversity and inclusion for ALL clients.
• Superb verbal and written communication skills.
• Compassionate and empathetic with a strong ability to establish and build trust.
• Organizational skills required to manage complex, deadline-driven paperwork.
• Proficient with Microsoft Office Suite, Adobe, or related software.

• Valid licensure to practice as a Licensed Vocational Nurse (LVN) in the State of California.
• At least 1-2 years of clinical nursing experience preferred.
• Knowledge of appropriate software including Microsoft Word, Excel, Outlook, Microsoft PowerPoint, and Adobe Acrobat.
• Bi-lingual in Spanish (preferred).
