The Care Coordinator, RN is a Registered Nurse who serves as an advocate and shares a commitment to excellence, innovation and compassionate care for patients with advanced heart failure and pulmonary hypertension. The Care Coordinator, RN collaborates with an interdisciplinary team to navigate the plan of care and facilitate the coordination of services across the health system. The role emphasizes teamwork, patient and family advocacy, program development and growth, and the use of critical thinking to analyze and implement processes that ensure quality outcomes, program development, and growth.

- Apply critical thinking to analyze processes and implement improvements to ensure quality outcomes and continued program development.
- Plan and prioritize nursing care to meet patient needs within the health system.
