- Assure completion of the RAI process from the MDS through interdisciplinary completion of the care plan.
- Initiate and monitor RAI process tracking, discharge/reentry, and Medicaid tracking forms through the EMR system.
- Follow up with staff as necessary to assure compliance with documentation standards.
- Complete patient assessments, data collection, and staff interviews as instructed by the current RAI manual.
- Facilitate accurate determination of the Assessment Reference Date that reflects patient care needs and resources for Medicare/Medicaid reimbursement.
- Provide interdisciplinary schedule for all MDS assessments and care plans as required by OBRA and PPS.
- Ensure appropriate signatures are obtained as required.
- Ensure the interdisciplinary team makes decisions on completing additional MDS assessments based on clinical criteria.
- Assist director of health services or designee with identifying significant changes, physician orders, and verbal reports to keep the MDS and care plan current.
- Coordinate scheduling, notice of patient care planning conferences, and communicate outcomes to staff, patients, and responsible parties.
- Help ensure all MDS information and care delivered are documented and supported.
- Assist executive director/ director of health services with monitoring that a care plan is initiated upon admission.
- Participate in daily stand‑up meetings and communicate needs for changes in PPS timelines and assessment reference dates.
- Relay and act upon information from Case‑Mix Specialist audits.
- Act as resource person for computer issues related to the MDS process.
- Maintain communication with consultants to ensure appropriate Medicare coverage.
- Attend daily and weekly Medicare or Utilization Review meetings as required.
- Correct and ensure completion of final MDS and submit patient assessment data to state and federal agencies.
- Assign, assist, and instruct all staff in the RAI process, PPS Medicare, Medicaid (Case‑Mix), and clinical computer systems.
- Ensure timely submission of the MDSs to the state with proper follow‑up on validation errors and maintain records in an orderly fashion.
- Graduate of an approved Registered Nurse program and licensed in the state of practice.
Minimum of one year of MDS experience.
- RN or LPN nursing experience in a skilled nursing facility.
- Excellent knowledge of Case‑Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
- Thorough understanding and experience with PDPM.
- Knowledge of Quality Indicators and Quality Measures.
- Familiarity with OBRA regulations and Minimum Data Set (MDS).
- Knowledge of the care planning process.
