Job SummaryWe are seeking a CLINICAL SERVICE NURSE II to be an integral member of the healthcare team and provide strong clinical and interpersonal skills and collaboration with management to oversee daily activities that support positive staff/employee/patient relationships.
Responsibilities- Coordinate with Heart Center access teams to ensure clinic schedules reflect appropriate patient appointments; obtain new patient records, organize, tab them for MD review; ensure return patients have necessary tests/labs; surgical clearance – send to necessary MD; schedule and reschedule makeup clinics.
- Coordinate the development and implementation of educational programs designed to convey specialized knowledge and skills to nursing personnel and patients. Support recruitment for HF clinical trials.
- During Remote Monitoring:
Download interrogation from device website, review for scheduled and unscheduled transmissions on urgent/concerning issues per protocol, and upload preliminary report into server (Scottcare). Assign report to provider. - Conduct pre-visit with patients if time allows and report out to provider; collaborate/coordinate with clinic staff to recheck vital signs as needed; educate patients regarding labs, procedures (Cardio
MEMS, Barostim, AHFT evaluations), medications, diseases; tee up orders for tests/procedures as needed; coordinate hospital admissions, queue all refills in clinic-AEMR and review with Providers. Provide education on new therapies for infiltrative heart disease and PAH. - Follow up to ensure labs/procedures are completed; discuss abnormal results with providers, call patients with explanation of abnormal results, annotate normal results and forward to patients. Coordinate labs/studies with outside facilities, obtain appointments with specialists and coordinate with patients, review all grid sheets after clinic to ensure items are completed. Communicate recommendations based on remote monitoring data (Cardio
MEMS, ICDs). - Triage for acute patient population independently, including manage incoming patient calls, coordinate plans with providers, and return patient calls including follow-ups and recommendations. Manage prescription refills as directed by provider. Complete prior authorizations, patient/insurance requested forms, assist with provider inbox management, and coordinate referrals. Manage logistics for geographically distant patients needing multiple tests.

- Work requires graduation from an accredited BSN program.
- Exception:
Registered nurses hired between July 1, 2014 and April 11, 2021 without a Bachelor's degree in Nursing (or higher) are encouraged to enroll in an appropriate BSN program within two years of their start date but must complete the program within seven years of their start date. Registered nurses hired before July 1, 2014 are not required to enroll in a BSN program to remain in this job classification. - Twelve months of appropriate clinical experience is required.
- Must have current or compact RN licensure in the state of North Carolina. BLS required.
The schedule for this position is day shifts, Monday-Friday, with no weekends or holidays.
