Primary Care Nurse Practitioner/Physician Assistant
Eventus Whole Health is seeking a Nurse Practitioner or Physician Assistant to provide primary care in skilled nursing and assisted living facility settings as part of an integrated care team.
OverviewEventus Whole Health delivers an integrated model of care to adults residing in skilled nursing and assisted living facilities through a network of healthcare providers including physicians, nurse practitioners, physician assistants, medical assistants and in-house support staff.
Essential Duties And Responsibilities- Provide primary-care services in accordance with Eventus Whole Health site-specific protocols and the standard of care for family medicine, geriatric medicine, and/or internal medicine as it pertains to long term care facility work.
- Review and sign MARs as requested by facilities; sign all orders specific for Primary Care while at the facility and/or within 24 hours if faxed or emailed, including phone orders, DME orders, Death Certificates, Plans of Care, PT/OT orders, hospice orders as appropriate, and other orders relevant to patient care; review and address labs within 24 hours of submission; manage narcotic prescriptions per facility patients and Eventus policy;
provide facilities with adequate time for paperwork during visits. - Provide on-call triage duties in accordance with the on-call triage protocol policy; adhere to the latest triage schedule as determined by Eventus Whole Health policies.
- Complete documentation and submit billing records to Eventus Whole Health per charting and bill sheet submission policies; provide a hard copy of electronic documentation within 48 hours of service for inclusion in the patient’s facility medical record.
- Establish and maintain open and positive communication with facility staff and administration; provide instruction or feedback regarding medications and pertinent caregiver information; meet with facility administrator or care coordinator before daily patient care visits and before leaving the facility.
- Be knowledgeable of and adhere to Eventus Whole Health standards, policies, procedures, and applicable laws and ethical standards; comply with all regulatory agencies governing health care delivery.
- Maintain patient confidentiality at all times, including appropriate use of cell phones, emails, text messaging, patient charts and EMR; comply with HIPAA rules and regulations.
- Consult with the clinical supervisor before refusing a resident, family, or facility request or recommendation regarding care.
- Masters degree in Nursing with current, valid licensure to practice medicine in the state of residence; all required certifications must be current and valid.
- Or graduate from an accredited physician assistant program; pass the national certification examination; hold a state license as a Physician Assistant; specialization in geriatric, family, or psychiatry is desirable.
