Hope Services, LLC is seeking a Care Manager within our Tailored Care Management Department.
Applicants must be a qualified professional (not licensed or provisionally licensed) and have experience with NC Tailored Care Management.
Primary Responsibilities- Promote whole‑person, integrated care by planning, coordinating, tracking, and closing gaps in care monitoring individuals.
- Assist individuals in achieving key goals of the tailored plan related to behavioral health, mental health, and physical health.
- Provide direct patient contact, including in‑home and community appointments.
The Care Manager shall report to the Care Manager Supervisor.
