Position Summary
The Dietitian provides comprehensive nutrition and dietetic services to pediatric and adult patients, including prenatal care, at Neighbor Health Center (NHC). The Dietitian works as part of an interdisciplinary Care Team to support the management and prevention of chronic diseases, promote healthy lifestyles, and improve overall patient outcomes. This position includes providing diabetes education and management support; certification as a Certified Diabetes Care and Education Specialist (CDCES) is strongly preferred.
The Dietitian works onsite Monday through Friday, 8:00 a.m. to 5:00 p.m.
- Provides medical nutrition therapy (MNT) for pediatric, adult, and prenatal patients, using evidence‑based guidelines.
- Participates as an integral member of the Care Team, collaborating with providers, nurses, behavioral health clinicians, and care coordinators to support comprehensive patient care.
- Develops and implements individualized nutrition care plans, including assessment, goal setting, counseling, and follow‑up.
- Provides diabetes self‑management education (DSME) to individuals and groups in accordance with ADA standards; maintains or pursues CDCES certification.
- Educates patients and families about nutrition principles related to weight management, hypertension, hyperlipidemia, prenatal nutrition, and other conditions.
- Documents all encounters accurately and promptly in the electronic medical record (EMR).
- Participates in quality improvement initiatives, chart reviews, and care coordination meetings to enhance clinical outcomes.
- Coordinates with external partners, community organizations, and public health programs as needed to address food insecurity and other social determinants of health.
- Maintains required continuing education and professional licensure.
- Performs other duties as assigned by the Chief Medical Officer.
Skills and Abilities
- Computer literacy in EMR systems, internet use, and Microsoft Office (Word, Excel, Outlook).
- Excellent interpersonal, communication, and counseling skills.
- Ability to work collaboratively in a team‑based, patient‑centered environment.
- Strong organizational and documentation skills.
- Cultural competence and sensitivity in working with underserved populations.
- Ability to cultivate and develop inclusive and equitable working relationships with co‑workers and community members.
- Ability to serve as an advocate for individuals of all ethnicities, genders, ages, and backgrounds.
- Bachelor’s or Master’s degree in Nutrition, Dietetics, or a related field from an accredited program.
- Registered Dietitian Nutritionist (RDN) credential required.
- Licensed Dietitian/Nutritionist (LDN) in the State of North Carolina.
- Experience providing MNT to pediatric, adult, and prenatal populations.
- Two or more years of clinical experience preferred, especially in primary care or an FQHC setting.
- Certified Diabetes Care and Education Specialist (CDCES) strongly preferred.
- CPR certification required.
- Vaccines as required.
- Sitting or standing for prolonged periods.
- Walking and moving about to accomplish tasks.
- Carrying or lifting objects up to 20 pounds.
