Referral Care Coordinator – South Light Healthcare

Pay Range

Base pay: $18.00/hr.

Schedule

Monday‑Friday 8:00 am – 4:00 pm.

Benefits

As a full‑time employee, you are eligible for medical benefits the first of the month, 30 days after your hire date. Options include a standard plan and an HSA plan with a $300 initial contribution and up to $25 per paycheck match. You receive 10 paid holidays and PTO hours that accrue at a rate of 10 hours per month. After 6 months, you may enroll in our 401(k) plan with a 4% match.

Additional benefits include company‑paid life insurance, short‑ and long‑term disability, in‑house training, and extra pay for working holidays.

Responsibilities
  • Provide superior customer service when communicating in person, via phone, or online with staff, service providers, hospitals, and the general public.
  • Answer incoming calls, provide information regarding treatment services and general inquiries, and transfer calls to the appropriate source as needed.
  • Oversee and manage online referral portals from hospitals and service providers and coordinate services appropriately.
  • Request and review required documentation to verify compliance, completeness, and accuracy of data.
  • Respond to all referral requests within 24 business hours of receipt of initial communication and document outcomes.
  • Utilize electronic medical record to establish, update, and maintain accurate client information.
  • Develop an active knowledge of available resources, and work to build and strengthen professional relationships with community providers.
  • Promote and model South Light’s core values—compassion, excellence, collaboration, inclusion, hope, and authenticity—to ensure quality care and an effective work environment.
Qualifications

Education and Experience
  • Minimum of HS diploma or GED.
  • Minimum one year of experience in front office, customer service, or a related field. Behavioral health experience is a plus.
Knowledge, Skills & Abilities
  • Demonstrated ability and willingness to learn, strong attention to detail, positive attitude, and flexibility in a fast‑paced environment.
  • Commitment to a strong customer‑service focus rooted in empathy and connection with clients.
  • Excellent time‑management and prioritization skills to handle and manage a high volume of inquiries.
  • Professional interpersonal and communication skills.
  • Ability to work independently and as a member of a team.
  • Organizational skills to prioritize workload, meet deadlines, and provide accurate, succinct documentation of activities.
  • General understanding of requirements for MCOs, Medicaid, Medicare, and third‑party payers.
  • Ability to interact effectively with people of diverse roles, backgrounds, cultures, and socioeconomic classes, including those in crisis or resistant or negative toward the organization.
