Overview
Epiphany Family Services seeks to hire a Tailored Care Manager Supervisor to lead a team of care managers to coordinate and monitor whole-person care for individuals with needs like Intellectual Developmental Disabilities (I/DD), Traumatic Brain Injury (TBI), serious mental illness, or severe substance use disorders. The role requires a clinical background, experience with complex populations, leadership skills, and knowledge of NC Medicaid policies and electronic health records.
Key Responsibilities- Team Leadership:
Supervise, train, and mentor a team of Care Managers to ensure they provide high-quality care and meet service requirements. - Whole-Person Care:
Oversee care plans that address physical health, mental health, developmental disabilities, substance use, and other unmet needs. - NC Medicaid Compliance:
Ensure the team's services comply with North Carolina Medicaid waiver requirements and state policies. - Integrated Care:
Promote integrated care teams and foster collaboration between providers to achieve better health outcomes for members. - Case Management Oversight:
Review cases, ensure appropriate member assignments, and monitor care to close gaps in services. - Community Engagement:
Work with community stakeholders, assess service needs, and identify gaps or opportunities to enhance services. - Quality Assurance:
Conduct case record reviews to ensure services are high-quality and develop new policies and procedures.
Education
A master's degree in a related field such as social work, psychology, nursing, or a human services area.
Licensure
A clinical license, such as LCSW, LCMHC, or LCAS, is required. Dependently licensed candidates are encouraged to apply.
Experience
Two or more years of experience in care management, case management, or care coordination, particularly with individuals who have I/DD, TBI, or severe mental illness.
Skills
Excellent clinical, interpersonal, organizational, and communication skills. Experience with electronic medical records and knowledge of the DSM-5 are beneficial.
