• An experienced PT, enthusiastic about providing functional rehabilitation in real life settings such as cooking in clients’ kitchens, navigating grocery stores, restaurants, outdoor activities, and workplaces.

• Perform evaluations and develop treatment plans.
• Regularly re‑assess the effectiveness of treatment plans, attend staffing meetings, and participate in family conferences.
• Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources.
• Communicate patients’ needs and progress to the treatment team, physicians, clients, and family members.
• Communicate with other disciplines to ensure collaboration, coordination of care, and enhance patient outcomes.

• Minimum of a bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted license as a Physical Therapist in the state where practicing.
• Current Basic Life Support or Cardiopulmonary Resuscitation certification in accordance with state regulations.
• Minimum of one year’s work experience as a physical therapist preferred.
• Demonstrated knowledge of rehabilitation techniques related to complex neurological injury.
• Effective and professional communication skills, both verbal and written.
• Ability to lift up to 50 pounds and transfer patients, which may involve lifting up to 100 pounds following safety procedures.
• Fine motor skills, visual acuity, and ability to walk or stand for extended periods.
• Physical requirements include pushing, pulling, bending, stooping, reaching, kneeling, and frequent positioning.
• Health screen or examination may be required prior to assignment and periodically thereafter, depending on specific location policy, local or state regulations.
