Care Management Clinical Educator

SummaryThe Care Management Clinical Educator plays a crucial role in the development and implementation of education programs within the care management department. This position involves working closely with healthcare professionals to ensure that best practices are incorporated into their daily practices.
Responsibilities- Develop and implement education programs that enhance the knowledge and skills of care management staff, staying up to date with the latest evidence‑based practices and incorporating them into the curriculum.
- Conduct engaging and informative training sessions covering topics such as patient assessment, care planning, documentation, and communication skills, and convey complex information clearly.
- Provide one‑on‑one coaching and mentoring to staff, observing performance, identifying improvement areas, and offering constructive feedback.
- Conduct new coworker orientation sessions, covering department policies, procedures, and expectations, and ensuring new hires have the necessary resources to succeed.
- Participate in quality improvement activities, analyzing data, identifying improvement areas, performing audits, and implementing strategies to enhance care quality.
- Evaluate the effectiveness of education programs, collecting feedback, analyzing outcomes, and recommending program improvements in collaboration with leadership.
- Collaborate with other educators, clinical leaders, and subject matter experts to develop comprehensive education programs through interdisciplinary meetings and research.
- Maintain accurate and up‑to‑date records of training participation, tracking attendance, documenting completion, and generating reports as needed.
- Bachelor's degree in a healthcare or business‑related field. Master’s degree preferred.
- Valid license as a Licensed Clinical Social Worker (LCSW) or Registered Nurse (RN) in North Carolina.
- Minimum of 2 years of experience in case management.
- Extensive knowledge and experience in care management practices.
- Strong presentation and communication skills.
- Ability to work independently and collaboratively.
- Excellent organizational and time‑management skills.
- Experience in developing and delivering educational programs.
- Familiarity with adult learning theories and instructional design principles.
