- Schedule:
Full time Monday-Friday 8am-4:30pm - Hourly Rate: $25.00/hour
- Evaluates behavioral, emotional, cognitive, spiritual, and social needs of clients and family members and consults with treatment team regularly for the development of and progress towards treatment goals in accordance with clinical policy and accreditation guidelines.
- Facilitation of individual sessions to develop treatment plans.
- Facilitates all clinical services for clients including individual and family sessions, therapeutic groups, and referral source/outpatient provider contact as applicable.
- Responsible for maintaining clinical record for clients with appropriate documentation of clinical services and client program engagement in ECR in accordance with program policy.
- Provides documentation and support for utilization review in a timely way and participates in UR process as outlined in program policy.
- Participates in aftercare plan development and outcome data process.
- Provides case management services for clients and families through coordinating transportation needs, providing individual and family case management sessions, assisting in development of aftercare plan, providing referral and aftercare provider contact, addressing outside provider needs.
- Conduct Level of Care Assessments as required.
- Completion of all required trainings as designated by the company and accreditation/licensing entities.
- Other duties as assigned.
- Preferred Bachelor’s Degree in human services or related field and at least 1-2 years of experience in substance abuse and mental health field.
- High school diploma or equivalent considered with 1-2 years of experience in similar work.
