Community Care is hiring a Nurse Practitioner for our Partnership Program

This is a full-time hybrid opportunity dedicated to our Racine and Milwaukee facilities.
***$10,000 sign on bonus***
Responsibilities- Documents and updates health history and findings, physical exam, functional status evaluations, diagnostic test and lab evaluations, mental health status and function evaluations, preventative care, and health reminders.
- Within their official scope and practice, delegates appropriate tasks to interdisciplinary team staff, and provides leadership and guidance. Acts as a resource to staff to ensure appropriate care and follow through and provides care planning assistance.
- Educates members and families regarding health maintenance and acute and chronic illnesses.
- Travel:
Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage. Would require travel between the Milwaukee and Racine sites.
- Masters degree in Nursing required.
- Wisconsin Registered Nurse/RN license required. Family, Geriatric, or Adult NP certification is required. DEA registration required. Advance Nurse Prescriber required. NPI and CPR & AED required.
minimum of 1 year recent clinical nursing experience with an at risk or elderly population.
Job Schedule- Monday to Friday, 40 hours a week: 8:00 AM – 4:30 PM
