Social Worker, Healthcare Nursing

Overview

Job Description We are seeking a dedicated and compassionate Social Worker to join our team.

• Maintain required documentation in adherence to established standards.
• Provide direct social work case management to the company's Program of All-Inclusive Care for the Elderly.
• Facilitate team management and maintain documentation standards.
• Regular local travel within Racine County, requiring use of a personal vehicle, valid driver’s license, and proof of insurance coverage.

• Master’s degree in Social Work required
• Current Wisconsin Social Work Licensure required
• Minimum of 1 year experience providing care or services for a frail or elderly population required
• 1–2 years case management experience with elderly and/or developmentally or physically disabled population preferred

The position is based in Racine, supporting Racine County residents. This position is Monday–Friday 8am to 4:30pm.

This is a Contract to Hire position based out of Racine, WI.

The pay range for this position is $23.00 - $35.00/hr.
