Position: Registered Nurse - Outpatient Behavioral Health/Primary Care - Racine Location

Position Summary

Reporting to the Clinic Office Manager, the registered nurse provides comprehensive nursing care to the behavioral health and primary care departments, including administering medications, performing treatments, triaging patients, and providing patient education. Responsibilities include accurately documenting patient information, supporting Medical Assistants, and managing the patient flow in the department. The RN performs additional support duties to ensure efficient operations and excellent patient service in alignment with OCHC policies and procedures.

The RN will work collaboratively with the Chief Medical Officer, Director of Behavioral Health, Primary Care Nurse Manager, and Behavioral Health Clinic Manager.

Essential Duties and Responsibilities

The essential functions include, but are not limited to, the following:

Clinical Care & Coordination
  • Provide direct nursing care, including obtaining patient vitals, clinical assessment, triage (in-person and phone), immunizations, medication administration, injection management (e.g., long-acting antipsychotics), and patient education to patients of all ages.
  • Support primary care providers and behavioral health clinicians in delivering integrated care plans.
  • Collaborate with medical assistants, care coordinators, and social workers to ensure continuity of care.
  • Monitor and manage abnormal test results, care gaps, and follow-up needs.
  • Respond to medication refill requests and assist with medication prior authorizations.
  • Assist in care transitions and discharge follow-up for high-risk patients.
  • Provide nursing support for patients receiving psychiatric services.
  • Support behavioral health crises under the direction of licensed providers.
  • Promote trauma-informed, recovery-oriented, and culturally responsive care.
  • Assists Medical Assistants with rooming processes and patient care activities as needed.
  • Maintains clinical supply inventory
  • Operates and maintains equipment relating to the provider’s specialty, such as EKG machines, Nebulizers, oxygen tanks, etc.
  • Maintains and updates clinical skills by attending staff meetings and in-services.
Patient Education & Advocacy
  • Educate patients and families on self-management, medications, and health promotion.
  • Encourage engagement in preventive care, screenings, and wellness visits.
  • Advocate for patient access to services and community resources.
Quality & Compliance
  • Document all nursing activities accurately in the electronic health record (EHR).
  • Participate in quality improvement initiatives and population health programs.
  • Coordinates and facilitates handling of sample medications and any communication needed to relay to the provider regarding changes in medications or updates. Maintains and oversees the sample medication inventory and processes.
  • Comply with FQHC, HRSA, and state/federal regulations, including infection control and confidentiality standards.
Minimum Qualifications (Knowledge, Skills, and Abilities)

Education & Experience:

Registered Nurse license issued by the state of Wisconsin. Bachelor's degree in nursing.

Community health experience is preferred. Experience working within a Behavioral Health Clinic or Primary Care Clinic is required.

Requires 3-5 years of experience in nursing that includes experience in hands‑on patient care.

Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA). May obtain upon hire based on department discretion.
