We’re looking for an experienced Registered Nurse to coordinate care for patients with chronic conditions in our clinic setting. You’ll help track gaps in care, arrange follow-ups, support patient education, and collaborate with primary and specialty providers to improve health outcomes.

• Hourly Rate: $46.18 – $65.84 (based on clinical pay scale)

• Full-time, benefited
• Standard clinic daytime schedule

• Active California RN License
• Graduate of an accredited Nursing program
• Current CPR certification
• 3+ years in acute care, urgent care, or outpatient/family practice
• Strong EMR skills + ability to organize, triage, and communicate clearly
