Fortcom Health Services is seeking a Per Diem Care Manager to support our Adult Foster Care (AFC) program. This flexible role is ideal for a licensed social worker looking to make a meaningful impact through community-based care while maintaining a schedule that fits their availability. The Per Diem Care Manager will conduct member home visits every two months, complete documentation, and collaborate with the care team to ensure members receive appropriate, person-centered support.
Key Responsibilities- Conduct initial and ongoing psychosocial evaluations of AFC members.
- Assess and document the safety and suitability of the home setting.
- Participate in the development, implementation, and review of individualized AFC care plans.
- Conduct scheduled home visits and complete detailed care management progress notes.
- Review AFC caregiver logs at least every 30 days.
- Assist with training, evaluating, and supporting AFC caregivers in collaboration with the RN.
- Coordinate care and services with other healthcare and community agencies (e.g., VNA, DDS, DMH, MRC).
- Respond promptly to changes in member condition and communicate with the AFC RN.
- Assist with discharge and transition planning as needed.
- Maintain accurate and timely documentation in accordance with program requirements.
- Bachelor’s degree in social work, psychology, human services, or related field.
- Licensed Social Worker (LSW or higher) in Massachusetts.
- Previous clinical experience working with elderly adults or individuals with disabilities.
- Strong communication, organizational, and interpersonal skills.
- Bilingual in Spanish.
- Ability to work independently while contributing to a collaborative care team.
- Valid driver’s license and reliable transportation.
- Must be able to travel to member homes across the assigned area.
- Ability to conduct home visits throughout assigned regions.
- Must be able to access homes that may not be wheelchair accessible.
- Requires the ability to walk, sit, stand, stoop, and occasionally lift or support members during visits.
