Pay Range: $18.66 – $27.99 per hour | Based on Relevant Experience
Schedule:
Full-time, Monday – Friday, with some variability based on facility and patient needs
The Role
As a Licensed Practical Nurse (LPN) supporting Nursing Home services at Quincy Medical Group, you will play a key role in delivering high-quality, patient-centered care to residents in long-term care settings. This role focuses on supporting providers, coordinating care, and building trusted relationships with residents, families, and facility staff.
You’ll work closely with physicians and interdisciplinary teams to ensure continuity of care, timely clinical support, and compassionate service in a familiar, community-based environment.
Primary Responsibilities- Provide direct nursing care to nursing home residents in accordance with physician orders and care plans.
- Obtain and document vital signs, patient histories, and clinical observations.
- Administer medications and treatments within scope of practice.
- Communicate changes in patient condition to providers and care teams.
- Assist with care coordination, follow-up needs, and documentation in the electronic medical record.
- Support chronic disease management and routine clinical assessments.
- Collaborate with nursing home staff to ensure continuity and quality of care.
- Maintain compliance with infection control, safety, and regulatory standards.
- Perform other duties as assigned to support nursing home operations.

- Active Illinois LPN license required.
- CPR certification required (available upon hire).
- Long-term care or nursing home experience preferred but not required.
- Strong clinical skills, communication abilities, and attention to detail.

- Ability to work independently while collaborating with interdisciplinary teams.
