Patient Outreach Specialist – Value Based Care
Location: Quincy, IL
Pay Range: $15.35– $23.03 per hour | Based on Relevant Experience
Schedule: Full-time, Monday – Friday (daytime hours)
Quincy Medical Group is seeking a Patient Outreach Specialist to support our Value Based Care initiatives by proactively engaging patients and helping remove barriers to care. In this role, you’ll connect with patients—primarily seniors—by phone to schedule important appointments, explain care needs, and guide them through next steps in their healthcare journey. You’ll work closely with providers, clinical teams, and operational partners to ensure patients receive timely, coordinated, and compassionate care.
This position is ideal for someone who enjoys meaningful patient interaction, problem-solving, and helping patients navigate the healthcare system.
Primary Responsibilities:The Patient Outreach Specialist conducts proactive outbound calls to schedule essential appointments, screenings, and follow-up care, while also managing inbound patient calls. You will explain the purpose of appointments, provide reminders, and address patient concerns to improve appointment attendance and continuity of care. This role requires identifying barriers such as transportation or scheduling challenges and connecting patients with available internal resources.
You will coordinate care across departments, reschedule appointments when needed, ensure accurate documentation in scheduling systems, and maintain up-to-date patient demographic and insurance information. Maintaining strict confidentiality and compliance with HIPAA guidelines is essential.

High school diploma or GED required
Experience in a healthcare or clinical setting preferred
Strong verbal and written communication skills with a professional, empathetic phone presence
Ability to manage high call volume while maintaining accuracy and quality
Knowledge of medical terminology and scheduling systems is a plus
