• Provide direct patient care under the supervision of a licensed Physical Therapist, ensuring safe, effective, and high-quality therapy services. Key responsibilities include the following.
• Promote Elara Caring’s philosophy, mission, and administrative policies to support quality patient care.
• Provide direct physical therapy services in accordance with accepted standards of practice and physician certification.
• Follow the established Plan of Care developed by the supervising Physical Therapist and promptly report any changes in patient condition.
• Adhere to all state practice act guidelines and regulatory requirements.
• Verify the Plan of Care prior to each visit and deliver care according to physician orders, assessment data, and established standards.
• Incorporate patient-specific goals from the Plan of Care into daily therapy interventions.
• Educate patients and caregivers on the safe and effective use of prosthetic, orthotic, and assistive devices such as canes, walkers, and wheelchairs.
• Instruct patients and caregivers on home exercise and therapy programs to support continued progress.
• Maintain accurate, timely, and thorough documentation of patient care.

• Physically demanding, high-paced clinical environment.
• Full range of body motion required, including lifting, transferring, and assisting patients.
• Potential to work irregular hours or on-call schedules as needed.
• Ability to sit, stand, bend, lift, and move intermittently.
• Must be able to lift 50–100 lbs.

• Graduate of an accredited Physical Therapist Assistant program approved by the American Physical Therapy Association (APTA).
• Current, unrestricted Physical Therapist Assistant (PTA) license in the state of practice.
• Six (6) months or more of experience as a Physical Therapist Assistant preferred.
• Willingness and ability to travel within the assigned branch or coverage area (up to 50% travel).
• Valid driver’s license, reliable transportation, and current auto insurance in accordance with state laws.

This position reports to the Branch Director, Clinical Manager, Clinical Supervisor, or designee.
