Telephone; 30min Response - Average Admissions While On Call: 3
Requirements
- Board Status:
Board Certified;
Board Eligible - Minimum 5 years of experience
- State License:
Illinois - DEA
Required:
Yes - CSR License
Required:
Yes - Will not consider IMLC
- No Malpractice or Disciplinary actions
- If accepted 24-month Case Log will need to be presented at start of credentialing process
