Canyon State Academy is located on a scenic 180‑acre campus surrounded by a football stadium, an athletic center (gymnasium, padded wrestling room, a fully equipped weight room & more), a Farm student housing for 300+ youth, a Thrift Store, Cafe and Barbershop open to the community. The Medical Coordinator, Licensed Practical Nurse or Vocational Nurse (LPN/LVN) works as a staff member of the Student Services team. Arrange and coordinate appropriate health services, including urgent, emergent and preventative health services, for children in care. This includes maintaining complete, confidential medical records for children, ensuring appropriate communication of health needs to case managers and clinicians, scheduling health care appointments, coordinating transportation to health care appointments, ensuring qualified interpretation and translation services are in place, and serving as a liaison for community health care providers.
Qualifications- High School diploma or equivalent
- Current Licensed Practical Nursing Credential from the state in which employed
- Minimum of two years of previous medical services administration, including demonstrated experience in operating and securing comprehensive health services programs
- At least 21 years of age
- Ability to pass a criminal background check, drug screen (no THC), physical, TB test, and child abuse central registry search
- Bilingual in Spanish (required)
