Main duties of the job

The Practice Nurse will provide high-quality, patient-centred nursing care within asmall general practice setting. Key responsibilities include long-term condition management (such as diabetes, asthma, and COPD), health promotion,cervical screening, immunisations, wound care, and routine treatment roomduties.

The role involves undertaking clinical assessments,delivering evidence-based care, maintaining accurate clinical records, and working in line with practice protocols and national guidelines. The Practice Nurse will work closely with GPs and the wider practice team to supportcontinuity of care and positive patient outcomes.

As this is a Welsh-speaking practice, the post holder mustbe able to communicate confidently in Welsh to support effective patient careand engagement. The role also includes contributing to infection prevention and control, safeguarding, and quality improvement activity within the practice.

We are a small, friendly,Welsh-speaking dispensing general practice serving our local community. Ourteam consists of two GP partners, a salaried GP, 2 Physician Associates, 2 Practice Nurse, phlebotomist, and a supportive administrative and dispensing team who work closely together to deliver high-quality, patient-centred care.

Job Summary

The Practice Nurse will work as part of the primary healthcare team to deliver high-quality, patient-centred nursing care to the practice population. The role includes managing long-term conditions, health promotion, preventative care, and treatment room duties in line with evidence-based practice and national guidelines.

As a Welsh-speaking practice, the ability to communicate confidently in Welsh is essential to ensure safe, effective care for our patients.

Key Responsibilities

Clinical Responsibilities

  • Provide nursing care including wound care, immunisations, cervical screening, and treatment room duties
  • Undertake long-term condition management clinics (e.g. asthma, diabetes, COPD, hypertension)
  • Carry out clinical assessments, monitoring, and reviews in line with practice protocols
  • Promote health and wellbeing through patient education and preventative care
  • Maintain accurate and timely clinical records using the clinical system
  • Work within professional scope of practice and recognise limitations, referring appropriately.
