Responsible to ESH Head and Director in all matters pertaining to the administration and programme planning of ESH On‑site services.
Recommend policies and practices for the efficient and effective operation and management of services and assist ESH, Lead and Director in implementing those policies and practices.
Responsible for achieving key performance indicators in the client reach, outcomes and impact, providing progress reports and assisting in data analysis for planning and reporting purposes.
Supervise the programme planning and delivery which include developing / implementing service elements that are evidence‑informed and clinically relevant for clients and families.
Oversee the enrolment of clients into the on‑site services.
Develop and work with the multidisciplinary team, ensuring efficient use of staff and volunteers.
Responsible for ground sensing to find out the needs of the clients/caregivers in the area (working closely with the Outreach Services Team) and which programmes to prioritise.
Plan client and caregiver training programmes, group activities and/or events
Conduct pre‑post or 6‑monthly programme evaluation, update clients’ progress, collate and analyse statistics, as needed.
Coordinate or conduct training for clients and caregivers in community living, social and emotional wellbeing, physical maintenance and vocational activities.
Ensure the safety and wellbeing of the clients.
Engage and partner referral agencies, community organisations and any other relevant agencies, for programming and events
Participate in events organised or supported by SPD and assist the organisation in developing strong networks with other Social Service Agencies (SSA), community organisations and funding bodies like MSF, SSOs, PA, NCSS, SG Enable.
Responsible for the booking of facilities within the CC and working with community partners to plan for social inclusion activities
Any other duties as may be assigned from time to time.
A Degree or Diploma in social sciences or related discipline
Fresh graduates are welcome to apply
Relevant experience (3 years or more) in managing programmes for people with disabilities would be advantageous, though not mandatory
Analytical, flexible, resilient, and able to see different perspectives and draw connectivity
Confident as an independent project/team lead who can foster good teamwork
Able to work on weekday evenings, Saturdays and/or Sundays
