As a Home Health Registered Nurse, you’ll perform comprehensive in-home assessments, develop and implement individualized care plans, and deliver clinical interventions such as medication management, wound care, IV therapy coordination, and post-acute rehabilitation support. You’ll educate patients and families on disease management, safety in the home, and symptom monitoring, empowering caregivers with actionable skills and resources. You’ll document care in electronic health records, communicate with physicians and interdisciplinary teams, and participate in case management discussions to optimize outcomes and reduce hospital readmissions.

Compensation, Schedule, and Start The assignment begins on 02/16/2026 and is structured for a multi-week duration with a guaranteed 40.0 hours per week. Weekly pay ranges from $2,265 to $2,424, depending on experience and case complexity. A sign-on bonus and housing assistance are available, with extension opportunities based on performance. The program provides 24/7 support from a traveling team of coordinators, clinical leads, and help desk resources.
