Overview
Colorado Mental Health Hospital in Pueblo (CMHHIP) – Social Work/ Counselor I. This position is full time and is open to current Colorado residents only.
Description of JobYou are required to work at your assigned work location during scheduled times. This position, on-site at CMHHIP, is scheduled Monday–Friday, 8:00 a.m. – 5:00 p.m. These working arrangements are subject to change. The position exists to provide, under an LCSW, therapeutic interventions including psychosocial therapies, individual, family and group treatment to severely emotionally disturbed/cognitively impaired patients and their families. This role completes assessments and disposition planning and coordinates interagency communication to ensure continued appropriate level of services following discharge.
It individualizes psychosocial therapy approaches to meet patient needs and provides training and consultation to other professional staff.
- Assessments:
Under the supervision of an LCSW, completes psychosocial assessments by interviewing, collaborating, analyzing, and assimilating social histories; generates a clinical summary and treatment recommendations that affect treatment and continuity of care in the community. Provide documentation as required. If the patient has been in the hospital for over 1 year, completes an annual psychosocial assessment. - Community Reintegration:
Coordinates discharge planning; evaluates patient needs related to community resources and mobilizes those resources. Serves as liaison between the hospital and community agencies; identifies appropriate community placements and treatment programs. Provides case management by making referrals to community agencies and maintaining relationships with Social Security, Social Services, Court system, jails, DOC, probation, parole, DYC, attorneys, CMHC, CCB, nursing, boarding, and group homes, individuals and families.
Attend meetings with listed agencies. - Treatment Planning:
Participates in direction and formulation of treatment plans; collaborates with the multidisciplinary team to design interventions that meet patient and family education and therapeutic needs. Provides required documentation. - Treatment:
Provides individual, family, group therapy and system interventions using social work theory and methods with attention to diversity. Provides necessary documentation regarding treatment. - Professional Development:
Keeps up-to-date with theories for assessment and treatment and works with team members to gather outcome data to assess treatment effectiveness. Provides necessary documentation. - Other Duties as Assigned:
Performs other duties as assigned.
Experience – Four (4) years of relevant experience in an occupation related to the work assigned to this position, OR
Education and Experience – A combination of related education and/or relevant experience equal to four (4) years.
Preferred Qualifications- Master of Social Work (MSW)
- Knowledge of casework methods and their applications
- Knowledge of human growth and behavior, basic sociological and psychological treatment practices
- Knowledge of interviewing skills and techniques
- Ability to express ideas effectively, orally and in writing
- Ability to exercise good judgment in evaluating situations and making decisions
- Standard Background check and Reference Check required
- State residency in Colorado at the time of application and throughout employment
- Motor Vehicle Check for state-owned vehicle use
- Valid Colorado Driver’s License or non-restricted U.S. Driver’s License for positions near the Colorado border
- Out-of-State Driver’s License holders must obtain a Colorado license within 30 days of hire
- Other program-specific conditions of employment
- Disclosures related to prior state employment actions as applicable
- Compliance with CMHIP training requirements (e.g., safety, defensive tactics)
- Ability to meet physical requirements and communicate effectively in English
- Travel required; TB screening and annual influenza vaccination
