We are seeking an Occupational Therapist (OT) to provide in-home, individualized therapy to adult clients in Pueblo, CO. You ll work directly with patients to help them improve independence, mobility, and daily function through personalized, evidence-based care.

• Full-Time or Part-Time schedules
• Competitive Pay: $70 - $85 per routine visit
• Start of Care Visits:
  Up to $120 per SOC

• Conduct comprehensive assessments and develop individualized treatment plans
• Deliver skilled therapy focused on ADLs, mobility, home safety, and functional recovery
• Educate patients and caregivers on home programs and adaptive strategies
• Document patient progress and update care plans as needed
• Collaborate with nurses, therapists, and other team members to ensure coordinated care

• A valid Occupational Therapist license in Colorado
• Proven experience in home health or a related field
• Excellent communication and interpersonal skills
• Proficiency in electronic medical record documentation
