About The Mental Health Transitional Living Homes
The CDHS Office of Civil and Forensic Mental Health’s Transitional Living Homes Program was created to increase bed capacity for persons in need of residential behavioral health treatment. HB 22-1303 authorizes new beds for residential services at a less restrictive level of care and to reduce the backlog of such persons in more restrictive settings. It directs the Department of Human Services and the Department of Health Care Policy and Financing to add at least 125 additional beds at mental health residential treatment facilities for adults, and to renovate a building at Fort Logan to create at least 16 additional civil beds.

These beds serve as a step-down option from inpatient care and as a step-up when medically necessary.

Description Of Job

About the Position

This position exists to provide direct hands-on milieu management care to clients, including assistance with activities of daily living, documentation of care, awareness of psychosocial and physical needs/safety, engagement in social and recreational activities, and support in the continued development of social and life skills. Services are performed in accordance with professional standards and regulatory requirements.

Responsibilities
  • Provide direct client care by hands-on performance and/or assistance with activities of daily living (ADL) and continued development of social and life skills through engagement in recreational and social activities.
  • Provide environmental needs for clients by arranging/adapting personal care items (food, clothing, etc.) and supporting ongoing needs.
  • Possibly guide and instruct clients to engage in self-care activities as part of developing ADLs.
  • Assist clients in becoming oriented to the program, including review of program requirements, client responsibilities, and inventory of personal items.
  • Supervise clients in care and ensure daily needs are met.
  • Monitor safety by supervising client whereabouts, interactions, and conditions; refer potential changes to the MHTL Homes team as needed.
Transportation
  • Transport clients to and from appointments and recreational/social activities when necessary.
  • Ensure safety in vehicles (seat belts, road rules, etc.).
  • Maintain a schedule of outside appointments and activities in coordination with the MHTL Homes team.
  • Complete trip logs timely.
Department Standards and Staff Development
  • Adhere to department standards and staff development requirements.
  • Follow MHTL Homes programming and operation standards per policies, procedures, and regulatory requirements.
  • Complete staff training, including MHTL Homes-specific and CDHS mandatory training.
  • Attend assigned meetings and in-services as scheduled by the supervisor.
  • Ensure documentation such as shift notes, incident logs, temperature logs, and other logs related to life, safety, and health are timely and accurate.
  • Act as a resource and orient new/pool staff related to milieu management and safety basics.
Medication Administration
  • This position requires certification as a Qualified Medication Administration Professional (QMAP). The MHTL Homes programs will assist non-certified individuals to obtain certification.
  • Administers medications per MAR; records medication times, doses, client responses, refusals, and related information; allows self-administration when applicable.
  • Dispenses medication and may provide topical medications as appropriate.
  • Review MAR, prepare medication, and provide to client with water as directed.
Other Duties
  • Other duties as assigned for business needs.
Minimum Qualifications , Substitutions, Conditions of Employment & Appeal Rights

Education
High school diploma or GED

AND
One (1) year of experience in an assisted living, nursing home, home health, or other congregate care/residential behavioral health setting
