Case Manager

Role Description

This is a full-time, on-site Case Manager position based in Providence, RI. The Case Manager will be responsible for coordinating person centered plans, supporting clients in accessing needed services, and tracking client progress to ensure goals are effectively met. Additional responsibilities include liaising with healthcare providers and community resources, advocating for client needs, and documenting case activities in compliance with organizational protocols.

As an integral part of a collaborative team, the Case Manager will work closely with other professionals to support clients' mental health and well-being.

• Case management, client advocacy, and service coordination skills
• Strong interpersonal skills, with the ability to build trusting relationships and work effectively with diverse populations
• Exceptional organizational, problem-solving, and time management abilities
• Proficiency in maintaining accurate records and preparing client reports
• Familiarity with community resources, social services, and mental health support systems is preferred
• Bachelor’s degree in Social Work, Psychology, or a related field
• Previous experience in case management or a related role is an advantage
• General familiarity with mental health care approaches like therapeutic methods and medication management would be beneficial
• Valid driver's license and reliable transportation for client visits as needed
