• Evaluates sick, urgent care, transitional care, and primary care patients
• Performs annual physical exams as needed
• Educates patients on maintaining proper health
• Evaluates, makes recommendations, co‑manages, and treats patients’ medical needs
• Participates in an interdisciplinary team providing care and coordination, ensuring the highest level of care
