Position: Case Manager Team 1B $2500 retention bonus

This position is eligible for $2,500 retention bonus.

Job Summary:
The Providence Center Case Manager Team 1B coordinates services for consumers to minimize stressors and to facilitate the reduction of psychiatric and substance abuse disorder symptoms. Encourage consumers to attain the highest possible levels of independence.

Duties and Responsibilities include the following. Other duties may be assigned.

  • Coordinate a variety of services.
  • Provide support counseling, problem solving, contracts and limit setting.
  • Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services, to include working with substance abusers, as it relates to case management.
  • Present verbally a synopsis of actual cases as required.
  • Arrange moves for consumers.
  • Accompany consumers to appointments with agencies, clinics, doctors, etc.
  • Initiate vocational/job development activities with consumer.
  • Make assessments and provide appropriate crisis interventions and risk management as needed.
  • Teach consumers adult daily living skills.
  • Integrate skills training in all aspects of Rehab. and Recovery Plan.
  • Maintain a clean, healthy and safe environment at all times.
  • Initiate treatment and advocate for consumer within network of care.
  • Schedule appointments for consumers within The Providence Center and with outside agencies.
  • Assist consumers with benefits acquisition (social security supplemental insurance (SSI), food stamps, advocates office, medical doctor).
  • Negotiate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.

    Transport consumers in own vehicle or Center van to planned activities and appointments.
  • Complete recordkeeping in accordance with Health Information Services (HIS) and other center requirements within required time frames and abide by center confidentiality policy.
  • Monitor confidentiality in accordance with Center policy and legal requirements.
  • Complete required reports and evaluations (MHPRR checklist, Psycho-social form).
  • Maintain cooperative relationships with Center staff, consumers, community agencies and the public.
  • Communicate with various local agencies and other departments or teams at The Center.
  • Work in cooperation with co-workers and supervisory staff.
  • Attend trainings, case presentations and conferences.
  • Serve on appropriate Center committees.

Requirements: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Associate's degree in Human Services or field required. Bachelor’s degree from accredited College or University preferred.

Experience in Human Services Field preferred.

Bilingual Spanish preferred.

Must have valid driver's license, registration, current inspection and insured automobile.
