Primary Care Integrated Behavioral Health Clinician
SUMMARY:
The Integrated Behavioral Health (IBH) Clinician works closely with our primary care teams to provide quick access to behavioral health assessment, triage, and brief treatment. The IBH clinician is embedded in the primary care practice and works directly with care teams to assess patients with behavioral health and/or substance use concerns. The scope of service includes, but is not limited to, brief assessment, crisis management, triage, and mental health/substance abuse treatment within the scope of their clinical abilities and training.

- Collaborates with primary care providers in recognizing, treating and managing mental health and psychosocial issues and acts as a contributing member to the primary care team.
- Triage both scheduled and unscheduled referrals from the medical team, treating some with brief treatment interventions and referring others to more appropriate resources.
- Provide brief treatment interventions for mild to moderate behavioral health concerns as well as crisis intervention.
- Consult in real time to assist patients and families with identified psychosocial issues and determining the appropriate level of care for patients.
- Conduct patient care in person, telephonically or via telemedicine for consultations and psychological treatment.
- Develop relapse prevention plans and help patients maintain stable functioning.
- Assist in the identification of “at risk” patients and in the development of plans to prevent escalation of existing conditions.
- Assist patients with self-management through education and telephonic/telemedicine engagement.
- Assist patients in navigating the healthcare system and serve as a liaison to external behavioral health care, including schools and other health care services outside of Brown Health Medical Group Primary Care.
- Assess the health care, educational and psychosocial needs of the patient/family.
- Participate in interdisciplinary care conferences with a focus on behavioral and social needs.
- Collaborate with the PCP, patient, members of the healthcare team including office care manager and care navigators, and required healthcare services outside of Brown Health Medical Group Primary Care.
- Ensure open communication regarding patient status with physicians, office staff and patients’ families.
- Attend required trainings and practice team meetings as scheduled.
- Perform other job-related duties as required.
- Provide behavioral health education and training to staff as needed.
- Licensed Independent Social Worker (LICSW), Licensed Clinical Psychologist, or Licensed Mental Health Counselor (LMHC) licensed in the State of Rhode Island and in good standing.
- Prior experience working in a behavioral health or medical setting with child clinical training.
- Ability to provide services telephonically and through telemedicine.
- Participation in ongoing quality assurance and improvement processes.
- Communicate compassion while maintaining equanimity.
- Ability to create systems and improvement processes as well as adapt to changing care processes.
- Excellent interpersonal skills and ability to work with and through others to achieve results.
- Maintain confidentiality with all aspects of information (including patient data) in accordance with the practice’s philosophy and policies as well as state and federal regulations.
