Position: Clinical Pharmacist - Home Infusion

Overview

Clinical Pharmacist - Home Infusion role at Brown University Health
. Reports to the Director, Home Infusion Pharmacy. Responsible for interpretation, evaluation, preparation and dispensing of medications pursuant to legitimate orders, in keeping with established policies, procedures, and standards of practice for Brown University Health Home Infusion Services. Collaborates with the multidisciplinary team to monitor, assess, and intervene to ensure safe, appropriate, and cost-effective medication use. Reviews, monitors, and verifies orders for oral and parenteral therapies, including chemotherapy, for appropriateness of drug, dosage, route, completeness, potential incompatibilities and drug interactions.

Identifies necessary changes to medication orders and documents them. Performs detailed in-process medication checks and ensures proper labeling and storage. Maintains compliance with regulatory requirements related to sterile compounding and hazardous drugs.);

Responsibilities
  • Collaborates with the interdisciplinary team and serves as a liaison for Brown University Health Home Infusion patients.
  • Adheres to the Home Infusion Patient Management Program (HIPMP) policies and procedures; supports patient participation.
  • Participates in developing and evaluating patient Plans of Care and progress toward goals.
  • Provides written education materials and verbal consultation to patients and/or caregivers regarding prescribed home infusion medications.
  • Ensures patient-specific parameters are included on the medication profile (e.g., diagnosis, allergies, weight, age).
  • Maintains current knowledge of compounding and sterile preparation principles, stability, compatibility, and regulatory requirements (RI DoH, RI Board of Pharmacy, USP standards).
  • Reviews orders for medications (including chemotherapy) for drug appropriateness, dosage, formulation, route, duration, and potential interactions.
  • Identifies and communicates appropriate changes in therapy to prescribers; performs detailed in-process checks for compounding.
  • Sets up, calibrates, and operates ancillary compounding equipment as needed.
  • Provides guidance on safety events, extravasations, hazardous drug handling, and spills; uses SDS appropriately.
  • Provides patient counseling using health literacy best practices; explains treatment regimens, precautions, and adverse reactions.
  • Oversees daily operations of home infusion pharmacy services, including aseptic processing environment, garbing, and cleaning procedures.
  • Mentors technicians, residents, and students; participates in orientation and training of staff.
  • Maintains documentation for drug purchasing, storage, preparation, and distribution according to policies and regulations; uses pharmacy systems for inventory and reporting.
  • Collaborates with purchasing to procure medications and supplies in a timely manner; engages in continuous quality improvement initiatives.
  • Maintains professional competence through staff development and professional organizations; performs other duties as assigned.
Qualifications
  • Education
    :
    Bachelor of Science in Pharmacy or Doctor of Pharmacy from an accredited college of pharmacy.
  • Licensure
    :
    Current unrestricted pharmacist licensure in Rhode Island (or 90-day temporary pharmacist license with plan to obtain full licensure before expiration).
  • Experience
    :
    Postgraduate year 1 (PGY-1) Residency or national Board Certification in Pharmacotherapy or Compounded Sterile Preparations preferred; clinical experience and interpersonal, organizational, and communication skills required. Knowledge of pharmacy information systems, automation, MS Word/Excel/Outlook, and ability to multi-task.
  • Credentialing
    :
    Obtain and maintain internal home infusion services credentialing within 2 months of start date and at least semi-annually thereafter.
Work Environment and

Physical Requirements

Extended periods of standing and walking; ability to operate a computer; visual and manual dexterity requirements consistent with laboratory and dispensing activities.
