The Internal Medicine Primary Care Clinician plays a crucial role in providing comprehensive healthcare services to patients, focusing on the prevention, diagnosis, and treatment of a wide range of medical conditions. This position requires a deep understanding of internal medicine principles and the ability to develop personalized care plans that address the unique needs of each patient. Clinicians will work collaboratively with a multidisciplinary team to ensure continuity of care and improve patient outcomes.

The ultimate goal is to enhance the overall health and well-being of the patient population while promoting a patient-centered approach to healthcare. This role is vital in fostering long-term relationships with patients and their families, ensuring they receive the highest quality of care throughout their healthcare journey.

Minimum Qualifications:

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
  • Board certification in Internal Medicine or Family Medicine.
  • Valid medical license to practice in the state of employment.
  • Strong clinical skills and a commitment to patient-centered care.

Preferred Qualifications:

  • Experience in a primary care setting.
  • Familiarity with electronic health record (EHR) system, Epic.
  • Bilingual proficiency in English and Spanish, ideal but not required.
  • Participation in quality improvement initiatives.

Responsibilities:

  • Conduct thorough patient assessments, including medical history reviews and physical examinations.
  • Diagnose and manage acute and chronic illnesses, providing evidence-based treatment options.
  • Develop and implement individualized care plans in collaboration with patients and healthcare team members.
  • Educate patients on health maintenance, disease prevention, and lifestyle modifications.
  • Maintain accurate and up-to-date medical records in compliance with healthcare regulations.
